Job summary

Whilst working as a Health and Well-being Nurse you will be working as part of multi-disciplinary team, delivering high quality specialist treatment to service users, in line with NICE-based protocols for the management of substance misuse.

The post holder will support community detoxes for both alcohol and opiate, enabling Service Users (SU) to gain recovery and exit treatment.

The post holder will be expected to deliver health and well-being reviews, harm reduction interventions, health promotion and be an advocate for SU to access long term condition support. They will promote active recovery and better health for all.

The post holder will assist in BBV screening and Hepatitis B vaccinations of SU, as part of the district wide harm reduction strategy, and assist in the development and targeted nursing interventions that aim to improve the management of long term conditions among this specific population. They will promote opportunities for recovery-based outcomes and sustained recovery from dependent and problematic substance misuse

The Health and Wellbeing Nurse will act as a liaison resource and provide support to colleagues in the delivery of alcohol or substance misuse interventions and treatments. Alongside championing public health initiatives.

Main duties of the job

Patient Care

  • Deliver high quality evidence based care.
  • Assess, plan, implement, evaluate care for SU.
  • Involvement in multi-disciplinary / inter-agency care planning to support care provision for those identified with complex health needs.
  • Support coordination of primary care, mental health and substance misuse services for the SU to empower their recovery
  • Conform to NMC professional codes of conduct.
  • To engage in Safeguarding as required.

Communication and Relationships

  • Maintain patient records in line with NHS standards.
  • Provide appropriate clinical information and assessment to support SU.
  • Work collaboratively with Humankind staff to deliver a recovery package for SU and enable progression in treatment and detoxes both alcohol and opiate.

Analytical, Planning and Organisational

  • Respond effectively to emergency situations which require a rapid nursing response intervention.
  • Analyse investigative results provide judgement on clinical problems which require further investigation, treatment or referral and develop appropriate treatment plans with SU.
  • Assess and support a SU who expresses suicidal ideation, liaise with appropriate services to support SU.
  • Determine appropriate response to crisis situations.
  • Manage competing priorities and react to unforeseen circumstances, incidents, and emergency situations.
  • Work within a demanding and high emotion environment with challenging patients which requires compassion and empathy.
  • Participate in audits.

The Health and Wellbeing Nurse will complete a nursing health assessment on all SU accessing support and treatment from NYH. This will include collecting information, completing investigations, interpreting results and liaising with others as deemed required.

Long term health conditions may require the Nurse devising and implementing a nursing care plan with the SU, with regular planned reviews.

The nurse will be provided with specialist CPD to gain competencies in alcohol detoxification and will play a key part in the monitoring of medicated detoxes.

The nurse will also be pivotal in the role out of depot opiate substitute medication ensuring it is administered in line with CD governance.

The nurse will work closely with the wider clinical team within NYH but also key healthcare partners - GP's, acute trust, Specialist nurses and EOL care to ensure safe, effective care is provided to our SU.
