Job summary

Working with the Community Falls Service team, you'll provide person-centered occupational therapy that helps people stay safe, independent and confident after a fall. Your work will support people to make informed choices about their daily activities and improve their wellbeing. You'll be part of a friendly, supportive team that values good communication, shared learning and high-quality care. You'll use evidence-based practice and work closely with colleagues across therapy, nursing and wider services to achieve the best outcomes for those you support.

This is an exciting opportunity to play a key role in delivering specialist, person-centred assessment and intervention for individuals who have experienced a fall, supporting them to remain safe and independent within their own homes and care settings across the Hull and East Riding area

The service operates across a 7-day working pattern, and there is an expectation to contribute to this, including weekends and public holidays. Main duties of the job

What you'll do:

  • Assess people's needs and plan safe, effective occupational therapy interventions
  • Deliver therapy programmes in homes and community settings
  • Provide clear, supportive advice to individuals, families and carers
  • Monitor and review progress, adjusting plans as needed
  • Contribute to service development, research and quality improvement
What we're looking for:
  • A Diploma or Degree in Occupational Therapy and current HCPC registration
  • Skills in assessment, communication and planning care
  • Basic IT skills and an understanding of relevant legislation and clinical guidance
  • Ability to manage your own workload and adapt to changing priorities
  • Willingness to learn, take part in supervision and develop your practice
