Job summary

You'll support unpaid carers through personalised assessments and practical help, making it easier for them to manage their caring role and stay well. As part of the Carers Information and Support Service, you'll work closely with colleagues across the "hub and spoke" model to make sure carers can access the right support at the right time. You'll build trusting relationships, communicate with compassion, and help carers navigate complex situations with confidence.

Main duties of the job

What you'll do:

  • Carry out Carer Assessments and agree support plans tailored to individual needs
  • Hold and manage your own caseload, keeping clear and accurate records
  • Work with partners and care providers to ensure coordinated support
  • Provide information, signposting and referrals, including Direct Payments where appropriate
  • Take part in service development, training and promotional activity
What we're looking for:
  • Level 3 qualification in Health and Social Care or equivalent knowledge and experience
  • Strong communication skills, including the ability to explain sensitive information
  • Ability to work flexibly, manage your own workload and respond to changing needs
  • Confidence using IT systems such as electronic records
  • A positive, inclusive approach and a commitment to the Seven Cs values
