Generic Support Worker

Job summary

This role helps people at risk of falls by supporting safe, timely care that improves confidence and independence. You'll work as part of an experienced integrated team, supporting fracture liaison nurses, therapists and other colleagues. You'll visit people in their homes and care settings, helping them get the right support at the right time. Your work will make a real difference to people's daily lives, especially those with complex needs.

Main duties of the job

What you'll do:

  • Register new patients and support the team to identify people who may benefit from the service
  • Carry out delegated nursing and therapy tasks within your assessed competencies
  • Record accurate patient information using electronic and paper systems
  • Support treatment programmes, observing and reporting changes in a person's condition
  • Assist with administrative duties to help the service run smoothly
What we're looking for:
  • NVQ Level 3 in Health and Social Care or equivalent knowledge and experience
  • Good communication skills and an ability to work with people with physical or mental health needs
  • Confidence in organising your own workload and working independently within clear guidance
  • Basic IT skills and an understanding of confidentiality and information governance
  • A team-focused approach and willingness to learn new skills
