Job summary
An exciting opportunity has arisen for a Band 4 Health and Social Care Assessor within the Bee at Home team. This role is primarily based at The Orchard Centre, where you will join a team of Health & Social Care Assessors to triage referrals in to the service, carry out patient assessments in their own home and coordinate the shifts, acting as a point of contact to the Health Care Assistants.
Main duties of the job
Bee at Home are a 7 day week service, operating between the hours of 8am - 10pm, covering Hull and the East Riding. You will be required to cover 3 long fays and 1 short shift.
Assessment experience is advantageous.

- Level 4 in Health and Social Care or equivalent level of knowledge, skills and experience
- Care Certificate or willingness to undertake this within induction period
Knowledge

- Knowledge of planning and completing initial client assessments
- Knowledge of policies, procedures and practices relating to domiciliary care
- Knowledge of a range of software programmes including Microsoft Word, Excel and Outlook
- Knowledge of completing risk assessments appropriate to domiciliary care
Desirable
- Knowledge of relevant legislation e.g. Mental Health Act (1983), Mental Capacity Act (2005), Safeguarding Adults and Deprivation of Liberty (DOLS)
- Clear understanding of domiciliary care and client needs in the community
- Knowledge of databases used within domiciliary care
