Main duties of the job

The role of the Health Advisor is to ensure that Patients/Carers receive the best outcomes this means providing the right care in the right place at the right time.

You will ensure that patient /Carer receives a Making Every Contact Count (MECC) assessment. You will coordinate and collate all incoming MECC referrals and make the necessary booking into the services where appropriate.

You will ensure you deliver a high quality patient centric experience within the Care Coordination Hub, supporting all other functions in delivering outstanding patient/Carer experience. To provide patient/Carer focused telephone and administration services that assist and direct patients/ Carers in accessing the appropriate service or healthcare professionals.
