Job summary
The opportunity has arisen to recruit to a band 6 Case Manager for the Beverley Hornsea and West Wolds Community Nursing and Conditions Team based at the East Riding Community Hospital. The post is a permanent position working hours 37.5 hours per week. The service covers between 8am and 8pm, shifts are rostered between these times and flexible working is supported in line with service need. This is an exciting opportunity for a reliable, flexible, enthusiastic and experienced community nurse who is committed to providing the highest standards of care and has proven leadership skills.
Main duties of the job
Applications are welcome from experienced registered community nurses who are able to demonstrate excellent communication and organisational skills as well as the ability to case manage an identified group of patients within a community nursing setting.
Flexible working over 7 days per week is offered within the service care delivery times of 8am and 8pm.

- Postgraduate qualification in a relevant subject e.g. Specialist Degree (nursing in own home or equivalent)
- Independent Non-Medical Prescriber or willingness to work towards

Knowledge
Essential
- Current Legislation and impact on community nursing services
- Evidence based practice
