Job summary

Would you like to work as part of our Intermediate Care or Community Rehab Therapy Team covering Hull and East Riding ?We are looking for someone who has experience in delivering rehabilitation, to join our friendly team of therapists.

The successful candidate will already have knowledge and experience in assessing and treating people who have physical rehabilitation needs. You will be supervised by registered Occupational Therapy and Physiotherapy staff, and will follow their treatment plans to deliver rehab programmes.

Main duties of the job

The main focus of the Intermediate Care service is to prevent hospital admissions and to facilitate hospital discharges, enabling people to stay at home safely, and to be able to manage their everyday activities .

The Community Rehab team work with patients to achieve their rehab goals in a variety of areas, including mobility and other activities of daily living.

Your role will involve visiting patients in their own homes in the community, or working with patients who are engaging in rehab in our bedded units.
