Job summary
We are looking for a highly skilled Advanced Practitioners to deliver effective clinical care to the patients within Venn PCN via our new home visiting service.
This is available as a Development Role for the right candidate.
This is a busy but extremely rewarding and vital role to support the housebound patients of VENN PCN to access safe and effective clinical care, in this role you will manage the care of a range of patients across all practices of Venn PCN.
Main duties of the job
- The post-holder will be required to complete system training and supervision to then work independently in the telephone triage of patients.
- After completing in-house competencies, the post-holder will be expected to work without direct supervision as an Independent Practitioner.
- After completing necessary clinical supervision, the post-holder will work as an independent practitioner providing face-to-face assessment, examination, diagnosis, treatment, management, referral, review or discharge, utilising a range of clinical skills and knowledge for patients presenting with undifferentiated and undiagnosed conditions, within the Competency framework.

Essential
- Registered 1st level Nurse or Qualified Ambulance Paramedic
- Maintain NMC/HCPC Registration and adhere to Code of Conduct
- Relevant post registration degree in either Advanced Care Practitioner/ Autonomous practice/ Emergency Care Practitioner
- Teaching/Mentoring qualification
- University modules completed in minor injuries and illness or willing to undertake
- Non-medical prescribing or willingness to undertake
- University modules completed to masters level or equivalent experience
Desirable
- Clinical based Masters degree in relevant subject or equivalent experience.
- Post Graduate University modules completed in minor injuries or illness.
Knowledge
Essential
- Comprehensive knowledge of relevant legislation, guidance and local initiatives and issues in relation to unplanned care
- Working knowledge of multi-disciplinary teams
- Evidence of Continuing Professional Development
- Comprehensive working knowledge of medication and interventions used in unplanned care

- Previous experience of using PGDs
