Job summary
We're look for a enthusiastic Link Worker to join our team. Social prescribing empowers people to take control of their health and wellbeing through referral to Link Workers who give time, focus on 'what matters to me' and take a holistic approach to an individual's health and wellbeing, connecting people to diverse community groups and statutory services for practical and emotional support.
Main duties of the job
The social prescribing Link Worker will work as a key part of the primary care network (PCN) multi-disciplinary team, to reduce health and wellbeing inequalities of the residents of St Helens.

Essential
- NVQ level 3 or working towards an equivalent qualification in a relevant subject area or demonstrable equivalent experience
- Demonstrable commitment to professional and personal development
- Training completed in motivational coaching/interviewing or equivalent experience
Knowledge
Essential
- Knowledge of the personalised care approach
- Knowledge of local communities, including the voluntary, community and social enterprise services provided.
- Knowledge of factors influencing behaviour and behaviour change approaches
- Have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the person needs is beyond the scope of the link worker role
- Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities, individuals, their families and carers.
- Have a specialist knowledge of one of the wider determinants of health (for example mental health, housing, debt/finance) and commitment to continually develop further knowledge, skills and expertise within this area
- An understanding of community development and asset based approaches, building on existing community and personal assets.
- An understanding of the barriers and challenges that people may face in connecting to and accessing local activities and support

- Demonstrable experience working with individuals within the community and/or within a healthcare settings
- Knowledge and experience of IT systems including the ability to use Microsoft office; Outlook, Excel, Word and Power Point and experience of accurate record keeping in line with company policies
- Experience of supporting people with their mental health, either in a paid, unpaid or informal capacity.
- Experience and ability to identify risk and assess/manage risk when working with individuals.
- Experience of developing a personalised care and support plan with an individual.
- Experience of working collaboratively with other health care and non-health care professionals to meet patient needs
- Experience of supervising staff/providing peer supporting/mentoring staff on a 1-1 and/or group basis.
- Experience in using coaching approaches and motivational interviewing techniques to support individuals to identify opportunities, challenge unhelpful thinking and support people's behaviour change.
