Band 6-7 Development OpportunityThe Hull and East Riding Integrated Nursing and Conditions Service has an exciting opportunity for a highly skilled, enthusiastic and innovative first level registered nurse to join the team to develop and deliver the new virtual ward concept and support the management of complex patients within the community setting.
The current service provides holistic high quality care for patients with a range of clinical/nursing needs across Hull and the East Riding of Yorkshire. Patient care is delivered within the patients place of residence including residential care homes and patients own homes.
You will be an integrated member of proactive community nursing teams working in partnership with associated health and social care providers, supporting and delivering care for patients with complex needs and will be involved in a new and exciting service development project establishing virtual wards in the community setting.
You must have the ability to work collaboratively with professionals from other organisations to facilitate integrated working. You must be an experienced practitioner, competent in the clinical assessment and care/management of patients and their families in challenging situations.
Main duties of the job
Candidates will need to be motivated and innovative with strong leadership and communication skills to support the development of community services. A flexible approach to practice, a commitment to ongoing learning and development and the ability to work autonomously are all essential for this post. An ability to commute within the organisation's geographical area, with access to a suitable means of transport, is essential.
Opportunities for development into this role may be obtained if the candidate meets the entry level essential requirements detailed within the Person Specification. In order to successfully complete development into this role it will be necessary to fulfil a detailed development plan which will be designed on an individual bases, following recruitment.
A candidate successful for development will be paid a band 6. Only on completion of the development plan will the successful applicant be confirmed into the position outlined in the advertised Job Description.
If the successful candidate already meets the essential criteria of the higher banded role, they can commence as a band 7.
