Job summary

Are you looking for a community Physiotherapy role? We are looking for a Specialist Physiotherapist to join our Community Rehab team based in Hull. We provide Physiotherapy assessment and treatment to adults who have a Hull GP. If you're looking for a role that is interesting , varied and challenging , this could be the role for you. Our caseloads are very varied , including Neuro, Falls, Frailty, respiratory and MSK housebound patients.

You would have good support from a Multi Disciplinary team ( we work closely with Occupational Therapists) and good professional support. We work collaboratively with external organisations , such as Hull Social Services, the Dept of Rehab Medicine based at Castle Hill Hospital, and voluntary groups. We have a Professional Development group for Physios and an in-service training programme. Our organisation is keen to support staff development, and offers excellent training opportunities.

Main duties of the job

    • To work with a multi-disciplinary team (MDT) delivering services to patients over the age of 18 who are registered with a Hull GP and are identified as in need of rehabilitation.
    • To support the line manager in the delivery of the physiotherapy service provided to their area of work on a day to day basis. To be involved in implementation of any change to work organisation/service delivery.

    • To assess and manage patients within the area of your work who may have complex and / or chronic presentation. To determine clinical diagnosis/need and physiotherapy intervention required. To maintain records as an autonomous practitioner in accordance with legislation and service policy.

    • To be responsible for the designated area of work, as agreed with the physiotherapy team leader, and to plan and organise efficiently and effectively with regard to patient management and use of time.

    • To prioritise and manage a large and complex clinical case load and delegate responsibilities to junior physiotherapists and assistants within the team as appropriate.

  • Competent in skills appropriate to the post e.g. Manual Therapy/Neuro
  • Evidence of CPD activity, including nationally recognised post graduate training in relevant speciality to Diploma level or equivalent
  • Evidence of reflective practice, IST and CPD portfolio
  • Autonomous practitioner works within codes of practice, policies and guidelines.
