Job summary

An opportunity has arisen for a qualified Practitioner to join City Health Care Partnership's Let's Talk Service. The service acts as a lead provider for a range of sub-contractor services and takes responsibility for assessment and initial treatment planning for a diverse population across Hull presenting with mild to moderate mental health issues, including depression, anxiety and psychological trauma. You will be required to provide initial assessment, risk assessment, and collaboratively identify the most appropriate treatment pathway. It is therefore essential that you can communicate sensitively and effectively with people from a variety of backgrounds and with those whose first language may not be English.

We are looking for qualified PWP's, RMNS', Social Workers, Counsellors, Psychotherapeutic Counsellors and Psychotherapists with experience of carrying out holistic mental health assessments. You will have excellent engagement skills.

The post will meet the demands of the provider element of the service this may require provision of face to face, telephone or computer-based communication.

Main duties of the job

The successful candidate will be offered appropriate training, support, and supervision to develop their skills and support the maintenance of their registration/accreditation. For candidates interested in a developmental opportunity and to expand their skills further where appropriate there may be additional opportunity to work rotationally across CHCP Services, which could provide opportunity to identify areas of interest. This could include working within prison or community settings or commencing IAPT therapy training posts.

Knowledge

  • Demonstrates an understanding of patient centred assessments and mental health clustering HONOS
  • Demonstrates an understanding of mild to moderate mental health issues and how it may present in Primary Care.
  • Demonstrates an understanding for the need to use evidence based psychological therapies and how it relates to this post

Desirable

  • An understanding of mental health clustering tool (HONOS)
  • Demonstrates a knowledge of the issues surround work and the impact it can have on mental health problems.
  • Knowledge of medication used in anxiety and depression and other common mental health problems.

Experience
