As the Care Coordinator, you will act as the primary point of contact for clients and support workers, providing critical operational support and overseeing daily care logistics.

Key Responsibilities

Client Care

• Develop and manage care schedules to meet clients’ needs, ensuring coverage for staff absences.
• Monitor and address cancelled care hours with Team Leaders.
• Oversee high-quality, person-centred care and maintain strong communication with clients and families.
• Host team meetings to promote collaboration and problem-solving.

Staff Management

• Recruit and match support workers to clients in partnership with recruitment.
• Collaborate with HR and Clinical Leads to address staff issues and ensure ongoing training.

Communication and Collaboration

• Coordinate with internal and external stakeholders to ensure seamless care.
• Supervise care and medication standards and maintain accurate records.
• Report on client progress and share service impact with stakeholders.

Operational Efficiency

• Plan and deploy resources efficiently across care packages.
• Support scheduling, billing, payroll, and participate in the on-call rotation.

About You

• Strong customer service skills with experience in domiciliary care or a similar environment.
• Outstanding communication and logistical planning skills.
• Ability to work both independently and as part of a team, with a calm approach under pressure.
• Mentoring and coaching experience to support the development of other team members.
• Own transport is essential.

Essential

• You will have strong customer service skills with experience in domiciliary care or a similar environment, outstanding communication and logistical planning skills, the ability to work both independently and as part of a team with a calm approach under pressure, and mentoring and coaching experience to support the development of other team members. Own transport is essential.
