Job summary

To provide high quality nursing care for patients requiring substance misuse services and detox through our programmes and pathways. This role is hands on providing clinical, psychosocial, and holistic solutions and contributes to the improvement, development, and innovation of our service offer to ensure our clients get the very best care.

Main duties of the job

Dispensing of controlled and non-controlled drugs to meet the clients physical support needs, ensuring a safe withdrawal from substance use.

As a nurse on shift patient care and wellbeing can require carrying out emergency anaphylactic and Naloxone administration if required.

Clinical and Psychosocial assessments to help create a person-centered integrated care plan.

Prioritise, organise and manage own workload in a manner that maintains and promotes Quality.

Complete accurate records of patient consultations.

Health promotions work for areas such as Sleep management, BBV, Wellbeing, Detox promotion and plan setting as well as new initiatives and monthly topics.

Joint multidisciplinary collaboration with all stakeholders including the wider nursing and community team.

Supporting colleagues and managers with new initiatives and service offers, and any reasonable requested Activity.

Care planning of a clients recovery journey which is person centered and puts the client at the focus of all decision making.

As shift lead, you will be responsible for the smooth day to day running of the shift; working alongside your multi-disciplinary team and ensuring operational responsibilities are completed for the safety and wellbeing of the patient group.You will complete new patient admissions jointly with the daily prescriber and plan a person-centred integrated package of care.Development and ongoing review of Care plans and risk assessments that are person centred, inclusive and flexible to the individuals presenting needs.Clinical observations to be carried out in a safe manner and understanding the appropriate escalation routes of clinical findings.Dispensing of controlled and non-controlled drugs to meet the clients physical support needs in line with the current policies and processes.Emergency anaphylactic and naloxone administration if required.

Delivery of KPIs in line with contract agreement to help evidence our service delivery.
