Job summary
The Recovery Healthcare
Assistant (HCA) role offers clinical skills and knowledge to be able to support
clients through there recovery journey, be that in a clinical aspect or
holistic. The purpose of this role is to offer high standards of care for our
clients, whilst being part of service development and innovation, to
continually improve our service offer.
The Recovery HCA role
is a hands on role that provides operational clinical and Psychosocial
support and advice in alignment with the Clients My Priorities plan and the
service client pathway. The Horizon service is integrated working as a
multi-disciplinary team so, it is important to ensure we have a focus on our
organisational objectives, as well as local service action plans. Delphi
recognises that people, through their skills, knowledge, experience, values,
and behaviours are integral to good Service performance and development. The Recovery
HCAs role through their professional skills and expertise will support the Horizon
team, to maximise performance, and in turn positively contribute to service development,
and ultimately the clients experience.
Main duties of the job
Work in a large, busy, cross site,
multi-disciplinary service. The Horizon team is a fast past team working with a
client group through the stages of their drug use, alcohol use and recovery.
Assess, plan and
evaluate care for patients, taking into account the potential needs of
individuals including young people & older people, with additional regard
to safeguarding, mental health, BBV, sexual health , smoking cessation and
toxic trio.
Clinical and Psychosocial assessments to help
create a person-centered integrated care plan.
Health promotions work for areas such as
Sleep management, BBV, Wellbeing, Detox promotion and plan setting as well as
new initiatives and monthly topics.
Delivery of KPIs in line with contract
agreement to help evidence our service delivery.
Joint multidisciplinary collaboration with
all stakeholders.
Supporting colleagues and managers with new
initiatives and service offers, and any reasonable requested activity.
Contributing to
client My Priorities planning of a clients recovery journey which is person.

Communication will be in a non-judgemental, empathetic way with patients, partner agencies and other professionals.

Clients will be engaged with the service throughout their care responding positively to interventions and exiting our service with improved health and wellbeing.
Clients will be supported and encouraged to build their personal strengths and social networks as part of their development to achieve identified goals.
New ideas will be offered to improve and strengthen our service offer so that we are able to provide the very best support to our clients.
Contract KPIs and deliverables will consistently be met or exceeded to ensure we are in the best possible position to retain the contract and win new work, enabling us to provide outstanding services to current and new clients.
Services and support will be delivered safely and effectively in line with all current regulatory requirements and internal policies and procedures.
