Job summary

This role exists to bring a new, more person centred approach to the care of patients engaging with the Community Mental Health Teams on The Fylde Coast. Through their own, personal, lived experience of Mental Health and various social issues, our Peer Support Workers provide a unique perspective to both service users and professionals. This role will help improve a range of social factors, including Mental Health, in collaboration with the wider, more traditional, Mental Health services. The role is a key part of the Community Mental Health Team offer and our teams lived experience provides real life examples of positive recovery, promoting a life worth living within our communities.

Main duties of the job

Prioritise and support a caseload of individuals.

Identifying recovery goals and supporting and empowering individuals.

Joint working with a number of multi-disciplinary teams.

Promoting and celebrating positive stories recovery.

Updating case notes on clinical NHS systems.

Facilitating and empowering peer led groups.
