Job summary

What you'll be doing.

You will be part of the Alcohol Team working in the hospital, our offices and the local community, carrying out assessments and initiating treatment for dependent alcohol users.

Liaising with Partner Agencies and the wider community services. Supporting clients to access physical, mental health and housing provisions along with referrals to recovery options and community activities.

Main duties of the job

  • Engage with, assess, and provide harm reduction and brief interventions to hospital patients.
  • Attend multi-disciplinary repeat attender meetings representing Delphi.
  • Book follow up appointments for all clients on hospital discharge and ensuring they are supported through the process.
  • Collate statistics around referrals, in-reach sessions and repeat attendees.
  • Link with hospital teams to ensure joint planning for hospital discharge.
  • Work in a large, busy, cross site, multi-disciplinary service.
  • Care planning of a clients recovery journey which is person centred and puts the patient or service user at the focus of all decision making.
  • Delivery of awareness sessions linked to substance misuse.
  • Planned continued detox where relevant.
  • Joint multidisciplinary collaboration with all stakeholders.
  • Delivery of KPIs in line with contract agreement to help evidence our service delivery.
  • Effectively deliver interventions to patients and service users.
  • Supporting colleagues and managers with new initiatives and service offers and any reasonable requested activity.
