Home Visiting - Advanced Nurse Practitioner ANP

Job summary

An exciting opportunity has arisen for an experienced ANP preferably with a prescribing qualification to join our Home Visiting Team , which has been implemented in the beautiful Derbyshire Dales. You will be an enthusiastic and motivated ANP who wishes to influence patient care in this bespoke area of primary care.

Your main duties will be to assist daily with mainly house bound patient home visits following a triage process that has been completed by the GP Practice that has responsibility for the patient care .

You will see on the day patients for acute and minor illness in dedicated appointment slots as part of the holistic home visiting service.

You will be employed and supported by the Derbyshire Dales Primary Care Network (DDPCN). You will work with, a diverse multi-skilled experienced professional team of clinical and non-clinical friendly and committed staff.

DDPCN covers Practices across the Derbyshire Dales area from the villages of Hope and Eyam in the north across to Tideswell, Bakewell, Darley Dale, and Matlock in the central areas down to the South Dales which includes Wirksworth and Ashbourne .

This post will be predominantly based in the Central and North Dales areas.

Main duties of the job

We are looking for a motivated individual who can work autonomously where appropriate to assist the clinical team with the completion of home visits and acute on the day patient consultations.

You will be Registered with NMC as RGN, with NMP qualification and evidence of masters level education for advanced clinician practice or working towards such a qualifaction which should include advanced examination skills.

The primary focus of the role is to work as an autonomous practitioner, primarily providing timely home visits to patients who are unable to attend the GP practice. It is essential that you can carry out appropriate physical assessments, establish accurate diagnosis, develop treatment and further management, or sign posting to other services within your scope of practice and level of qualification.

You will be supported by the Duty team to effectively manage your individual workload and ideally you will hold a prescribing qualification.

You will be assisted by an office based Duty Care Coordination team on a daily basis who will support the effective management of your individual workload .

You will be further supported by a Lead Community GP that is embedded in the team and weekly by the Clinical Director, plus the wider PCN Back office administration team .

Ideally we are looking for 30 to 34 hours per week (Monday to Friday ) and will consider job share /part time providing the needs of the organisation are met by a job share approach .

Demonstrating a high level of practice and knowledge whilst participating in direct evidence-based patient care. You will be a part of a multidisciplinary team providing professional and clinical support in the care of patients. You will provide holistic and quality care to our patients within the community. Looking for a practitioner with strong communication skills who is able to speak with patients in a way that puts them at ease while answering questions in understandable ways. Someone who exhibits independence, uses discretion, and has sound judgment.

Advanced Nurse Practitioner responsibilities include

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice

Work with patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

Communication

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background, and preferred ways of communicating

Anticipate barriers to communication and take action to improve communication

Maintain effective communication within the practice environment and with external stakeholders

Act as an advocate for patients and colleagues

Ensure awareness of sources of support and guidance (e.g. PALS) and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate

Managing information

Use technology and appropriate software as an aid to management in planning, implementation, and monitoring of care, presenting and communicating information

Review and process data using accurate Read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes

Requirements and skills

Must hold current license/registration to practice

Excellent verbal and written communication skills

Must have the ability to exercise sound judgment and discretion

Ability to maintain highly confidential information

Masters Degree in Nursing.
