Job summary
Following the national release of the ARRS GP
information & funding, Derbyshire Dales PCN is looking to recruit a newly
qualified, (within 2 years of CCT date & not having held a substantive post
before) enthusiastic & forward-thinking GP to join our Home Visiting
Service on a permanent contract .
You will be required to work 5/6
sessions across the areas that the DDPCN HVS supports.
DDPCN works in partnership and
provides the home visiting service (Monday to Friday) for the seven GP
Practices within our locality namely:
Evelyn Medical Centre, Hope
Eyam Surgery
Baslow Health Centre
Peak and Dales Medical Partnership (Bakewell &
Tideswell)
Darley Dale Medical Centre
Imperial Road Group Surgery
Lime Grove Medical Centre
We are a very supportive PCN &
will support you to work across the different surgeries and will have a lead GP
/Clinical Director as a mentor.
This is a great opportunity to get to
know our practices working across the north and central Derbyshire Dales areas
and will provide the successful candidate with varied experience.
As part of the role there may be opportunity to be seconded for short periods to the GP Practices in our PCN Family to maximise skill development in a GP setting , not just the Home Visiting Service . This option is currently under review and development and can be discussed further at interview .
Main duties of the job
To perform as an autonomous general practitioner working
independently and with other health care professionals to assess, diagnose and
treat the conditions of patients that have been triaged at practice level and
require an acute /same day home visit or a proactively assessed visit that
falls outside of the urgent criteria.
To provide expert professional advice to patients, carers
and colleagues and ensure the maintenance of clinical excellence. To be
involved in the provision of education and training of other staff.
To be professionally accountable and responsible for all
elements of clinical practice in accordance with the GMC code of conduct
ensuring the safety of patients and quality of their care is primary.
The post holder will need to be capable of independent
travel across the Derbyshire Dales area and work flexibly to meet the needs of
the Business.

Derbyshire Dales PCN is made up of seven GP Practices with a
weighted list size of 52988.
Based in Derby and Derbyshire ICB ,the network covers the
areas of Hope , Bradwell , Eyam
,Tideswell , Baslow , Bakewell , Darley Dale , Ashover , Youlgreave and Matlock
.
There is an excellent road infrastructure and access to and from the M1, A6 and A38 is easily achievable .

will make him/her-self available to undertake a variety of duties including
consultations, telephone consultations and queries, visiting patients at home,
checking and signing/authorising repeat prescriptions and dealing with queries,
paperwork and correspondence in a timely fashion.
Making professional, autonomous decisions in relation to
presenting problems, whether self-referred or referred from other health care
workers within the organisation
Assessing the health care needs of patients with
undifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signs
of illness
In consultation with patients and in line with current
practice disease management protocols, developing care plans for health
Providing counselling and health education
Admitting or discharging patients to and from the caseload
and referring to other care providers as appropriate
Recording clear and contemporaneous consultation notes to
agreed standards
Collecting data for audit purposes
Compiling and issuing computer-generated acute and repeat
prescriptions
Prescribing in accordance with the practice prescribing
formulary (or generically) whenever this is clinically appropriate
The post-holder will be required to visit patients in an
emergency if clinically indicated.
The post-holder will undertake fair share of home visiting
sessions per week as allocated.
