of the Multi-Disciplinary Tier 3 Bariatric Service.
Main duties of the job
Main duties will involve working
within Tier 3 services as part of a skilled multi-disciplinary team. You will
collaborate closely with healthcare professionals such as nurses, dietitians,
physicians, surgeons, psychologists, and administrative staff to assess,
provide interventions, and monitor patients from diverse geographical areas.
Clinical Responsibilities:
- Conduct comprehensive assessments of patients
referred to the Obesity clinic to determine suitability and create personalized
care plans.
- Plan, implement, and evaluate patient care within
the nurse-led clinic while adhering to evidence-based practices.
- Manage patients within the obesity treatment
program alongside the multidisciplinary team, following established protocols.
- Offer nutritional, motivational, and health
promotion advice to patients in clinic settings and over the phone.
- Support patients referred for bariatric surgery pre
operation, ensuring compliance with guidelines.
- Perform venepuncture when necessary and take
appropriate follow-up actions based on results.
- Communicate effectively and empathetically with
patients, addressing barriers to lifestyle changes.
- Assist in clinical data collection for audit
purposes and maintain accurate medical records.
