The role will be part of the Liaison Care Delivery Team completing Continuing Healthcare assessments and reviews.

The post holder will be undertaking assessment and reviews remotely for individuals in Care Homes and in the community.

The Care Practitioner will chair the Multi-Disciplinary Team (MDT) meeting as the Lead Assessor, the NHS Continuing Healthcare framework 2022 for the Decision Support Tool (DST) and subsequent reviews, and provide person centred evidence-based recommendations for eligibility.

As a practitioner you will be expected to challenge and report poor practice and discriminatory attitudes and behaviour relating to individuals you assess should you identify this during your review. This will include the reporting of safeguarding concerns as per the appropriate Integrated Care Board (ICB), Health Board and Liaison procedure.

Main duties of the job

Completing CHC reviews

Manage relevant follow up calls and request further documentation and evidence as required from other professionals and care providers.

Working to both Liaison Care and the ICBs Key Performance Indicators (KPIs).
