Modality LLP is a
large-scale provider of NHS Outpatient Services, renowned for its innovative
approach to delivering efficient pathways for patients requiring specialist
care, leading to high quality patient outcomes. We work in partnership with a
number of hospitals across the country to collaboratively redesign outpatient
services with more care being delivered in community settings.
We are looking
for an experienced Prescribing Clinical Pharmacist to expand our community Cardiology team and
support the growth and development of our service.

Main duties of the job
The post holder will support Modality LLPs
community Cardiology Service as an independent prescribing Clinical Pharmacist,
who acts within their professional boundaries, supporting and working alongside
a team of Cardiology specialists.
The poster holder will:
Work as part of a
multi-disciplinary team in a patient facing role. The post holder will take
responsibility for areas of disease management and undertake patient and medication reviews, to proactively manage
patient symptoms.
Ensure that the
service integrates with community and hospital pharmacy to help utilise skill
mix, improve patient outcomes, ensure better access to healthcare and help
manage workload. The role is pivotal to improving the quality of care and
operational efficiencies so requires motivation and passion to deliver
excellent service within the community setting.
Already
be a non-medical prescriber but will be supported to develop their scope of
practice in line with the needs of the service and their own training plan, as
agreed with the service clinical lead.

- Delivery of 1st consultations within a maximum of 4 weeks from referral from a choice of community locations, 7 days per week
