Job summary

You will be a Smoking Cessation Advisor for Dudley's new Integrated Health Improvement Service.

Your key responsibilities will be:

Provide evidence-based smoking cessation delivery across Dudley.

Delivering support in the areas of most disadvantage and highest need

Continuously develop methods and resources that ensure smoking cessation is embedded across the integrated service.

You will be a motivated, passionate, organised and dedicated deliverer, with significant experience in smoking cessation delivery. ABL take a strengths-focused, asset-based approach to community delivery, looking to innovate and partner, not replicate existing delivery. The role will require dynamic and flexible working, which embraces the vision and values we wish to establish for the Integrated Health Improvement Service around smoking cessation, ensuring an individual strengths-based approach to behaviour change.

Main duties of the job

This post will work under the line management of the Head of Service. The role will be varied, challenging and rewarding as you support and develop the service through implementation, delivery and ongoing innovation and service development.

Your Health Dudley (Integrated Health Improvement Service) overview

ABL Health, delivers the Integrated Health Improvement Service titled Your Health Dudley, which is commissioned by the Dudley Council. This transformative service seeks to work collaboratively with communities to support transforming services for the local population. The service will provide integrated behaviour support for smoking cessation, weight management, alcohol reduction, physical activity, cooking, family coaching and community health checks, all embedded in sustainable behaviour change and positive wellbeing.

Key responsibilities will be:

Motivate and empower clients to make behaviour changes.

Explain to clients the principles of the nicotine and Stop Smoking Service to enable them to make an informed choice of quitting.

Deliver evidence-based stop-smoking interventions, these include one to one, drop in, group sessions, telephone or on-line support, these must be in line with the NCSCT Standard Treatment Programme.

Work with other professionals to embed smoking cessation as part of routine care.

Provide a high quality, efficient and cost-effective services throughout a diverse range of community settings.

Lead peer support group sessions for successful quitters following intervention to prevent relapse.

Provide evidence-based education in all aspects of smoking cessation and behavioural change as well on-going motivational support.

Work with individuals to set realistic sustainable goals.

Ensure all client and service records (electronic or paper) are up to date, accurate and satisfy the ABL Health documentation policies.

To engage and recruit potential clients at promotional and community events.

Provide administrative assistance to the project in terms of client contacts, appointment bookings and confirmations.

Work as part of a multi-disciplinary team.

To conduct stakeholder engagement, identify and strengthen referral pathways and identify potential co-working opportunities working closely with district community engagement officers and project lead.

To attend stakeholder meetings as required

To attend relevant meetings as required, including regular Team meetings, smoking cessation meetings.

To attend training as required and keep up to date with the wider aspects of Tobacco Control and the wider determinants of health.

Assist with step-down approach identifying, coordinating & recording signposting to Tier 1 / community activity programmes.

Develop website / social media materials for smoking cessation and reply to forum queries on the website accordingly.

Be innovative in ways of delivering activity and open to new ideas.

Engage as and when required with local stakeholders to ensure robust referral pathways.

Ability to cover sessions as required due to sickness or annual leave.

Maintain a specialist clinical caseload as agreed with the Smoking Cessation Lead
