Job summary
ABL Health and Nottinghamshire County Council are partnering to deliver an Integrated Wellbeing Service across Nottinghamshire called Your Health Notts. This transformative service will play a central role in system change, bringing previously separate lifestyle behaviour change services together into one holistic service. It seeks to work collaboratively with communities to support the transformation of services for the local population. The service will provide integrated behaviour support for smoking cessation, weight management, alcohol reduction, physical activity and falls prevention, all embedded in sustainable behaviour change and positive wellbeing.
Main duties of the job
Your key responsibilities will include delivering ABL Healths family weight management programme through group and one-to-one sessions in the community and via digital platforms. You will implement healthy lifestyle education in schools, support families with toddlers experiencing selective eating, and deliver nutrition and physical activity sessions in various settings. The role requires a non-judgemental, client-focused approach, using motivational interviewing and evidence-based guidance. You will contribute across all integrated services, including adult weight management, alcohol reduction, falls prevention, and smoking cessation, while managing caseloads, monitoring client progress, and conducting assessments. Additional duties include supporting safeguarding processes, coordinating resources, maintaining accurate records, and promoting SMART goal setting. Youll also engage in community outreach, stakeholder meetings, service development, and social media support. Ongoing responsibilities include attending team meetings, contributing to performance reports, developing resources, supporting new staff, completing required training, and traveling flexibly across the project area.
