Job summary
ABL Health and Nottinghamshire County Council are partnering to deliver an Integrated Wellbeing Service across Nottinghamshire called Your Health Your Way. This transformative service will play a central role in system change, bringing previously separate lifestyle behaviour change services together into one holistic service. It seeks to work collaboratively with communities to support the transformation of services for the local population. The service will provide integrated behaviour support for smoking cessation, weight management, alcohol reduction, physical activity and falls prevention, all embedded in sustainable behaviour change and positive wellbeing.
Main duties of the job
Your key responsibilities will include delivering informative and engaging one-to-one weight management programmes for adults, covering healthy eating, behaviour change, and exercise, as well as providing safe, personalised advice to children, young people, and families. You will deliver evidence-based education on healthy lifestyles, following ABL Healths programme curriculums, using a client-centred, non-judgemental approach that incorporates positive psychology and motivational interviewing techniques. Responsibilities also involve conducting anthropometric and wellbeing assessments, supporting the triage process for the Integrated Wellbeing Service, and maintaining an active caseload by monitoring and motivating clients through a range of methods, including face-to-face, digital, and telephone support. You will deliver health promotion activities, contribute to training new staff and volunteers, accurately record client data, and uphold ABL Healths policies and procedures. Additional duties include developing and utilising curriculum resources, supporting ongoing service development, building community links to encourage new referrals, working collaboratively with the multidisciplinary team to improve health behaviours, and maintaining a high standard of personal and professional development in line with clinical governance frameworks. You will also be expected to organise your workload effectively and contribute to team and company meetings.
