The post
holder will work at St Josephs Hospice predominantly on the wards with input
to the community team mainly on call.

Main duties of the job
Delivery of medical
care to specialist palliative care inpatients with consultant supervision
(using a holistic approach, addressing the patients physical, psychological,
social and spiritual needs).
Work as part of the multi-disciplinary,
specialist palliative care team to optimise patient care.
Clerk
new admissions to the hospice through history and physical examination and
explore advance care planning.
Attendance at & participation in the
weekly Consultant Ward Round and Multi-Disciplinary Team meeting.
Communicating with patients and their
families pro-actively, providing appropriate information and support as
necessary.
Chairing
of family meetings when appropriate.
Use of the multi-lingual advocacy service
to aid assessment of and communication with patients & families who do not
speak English fluently.
Appropriate
communication with health professionals outside of palliative medicine, to
ensure good continuity of patient care.
Timely provision of medical letters and
discharge summaries.
Participation
in bereavement follow-up as required.

St. Josephs is a charity established in 1905 by the Religious Sisters of
Charity to offer patients diagnosed with a life limiting illness a range of
care and support. It has a national and international reputation for providing
high quality palliative and end of life care to a significant, growing number
of people living in East London and beyond, and has a strong reputation for

large team of clinical staff, working across community, in-patient and
out-patient settings delivering individualised, responsive and holistic support
to patients and their families/carers.
St Josephs
Hospice provides care for patients with a variety of advanced progressive
conditions. The proportion of referrals for patients with non-malignant disease
is currently 35%. Inpatients are
admitted to St Josephs Hospice for symptom control, terminal care,
psychological support and respite. Average length of stay is 16 days, with 50%
of admissions being successfully discharged.
