This post is for a full-time post 37.5 hours over 5 days per week, although 30 hours over 4 days per week would be considered. (Full-Time equivalent to 37.5 hours over 5 days per week/ Salary Band 6 £40,448.00 - £47,965.00 Pro-Rata, including HCAS).
An opportunity has arisen to join the Social Work Service
at St Josephs Hospice in Hackney. You will work as part of committed and creative team, which
offers innovative social care interventions and support to people with
palliative and end life care needs, and to their families, carers and children. The social work team works in
collaboration with other clinical professionals and volunteers across all areas
of the service; inpatient, outpatient and community.
Ideally with some knowledge of
palliative care, the Social Worker post requires an enthusiastic, confident and
innovative, Social Work England Registered Social Worker with some grounding in statutory
work. As a member of the team you will work with your senior social work,
welfare rights, carers coordinator and social work support colleagues to
provide seamless care for our patients and their families both within the
hospice and in the community.
Main duties of the job
You will need to have excellent communication, organisational and social work assessment skills. We offer you the opportunity to develop or enhance specialist Social Work skills through working with individuals and families facing advanced disease, death and bereavement. You will develop a clear understanding of the social, psychological and spiritual needs of individuals and their families, and of working within a hospice setting.
The successful applicant will have experience of working with and alongside people with complex social care and/or healthneeds, providing information, support and episodic social care interventions. You will have highly developed communication skills and be committed to personalisation and service innovation. You will be skilled at engaging and enhancing our relationships with local providers and partnerships to help ensure our services meet every persons needs.
