Job summary
We at Connect Health are delighted to be growing our team of talented clinicians and are currently looking for a Senior Clinician in Pain who is passionate about providing high-quality patient-centred care to join us as part of a large multidisciplinary team in Lincolnshire at either our Skegness or Grimsby sites.
Our Lincolnshire service is a community persistent pain service that provides a single point of access for NHS patients enabling a seamless and efficient patient journey. You will be part of a large, dynamic multidisciplinary team which includes Clinical Psychologists, CBT Therapists, Psychotherapists, Nurses Physiotherapists, Advanced Physiotherapy Practitioners, Occupational Therapists, Pharmacists, GPs with special interest in Pain, Consultants in Pain Medicine The community service design allows clinicians to work together to deliver seamless and evidence-based care for patients and enables inter-disciplinary learning.
We deliver our services out of a number of different hub site clinics and GP practices across Lincolnshire. This encompasses 14 sites across the localities of Lincolnshire CCG which includes Lincolnshire East, West, South Lincolnshire and South-West Lincolnshire. We are looking for applicants who are able to work from 2 or more of our sites, and we offer flexibility around where clinicians are based.
Main duties of the job
You will utilise Senior Pain Clinician expertise to function as a Pain specialist, managing a varied and complex clinical caseload within the community service contract. This will include:
- Determining clinical diagnosis and provide treatment indicated as an autonomous practitioner, developing appropriate packages of care, giving specialist clinical / professional advice to patients, carers, and other Health Care Professionals.
- Undertaking comprehensive assessment and diagnosis of patients with a wide variety of physical, and emotional needs. To formulate, deliver, review and progress individual treatment programmes, ensuring effective treatment, specialist advice and timely discharge is achieved.
- Maintaining patient documentation, records, and accurate statistical information to reflect care provided and to meet professional and organisation standards as well as local and national guidelines.
- Demonstrating a sound understanding of Clinical Governance, including ability to audit own practice.
- Being responsible for organisation, prioritisation, and planning of own caseload with support from our patient care co-ordination centre (PCC).
- Demonstrating clinical competencies to required company standards for a Senior Pain clinician, including providing support to Pain clinicians within the service
