Job summary

We at Connect Health are delighted to be growing our team of talented clinicians and are currently looking for a Health and Wellbeing Coach who is passionate about using their current skills supporting people to reach their own health and wellbeing goals.

This role is with Connect Health Pain Services (CHPS) community pain service serving the Durham and Darlington locality. The successful candidate will work to coach and motivate patients, to guide them through multiple sessions, supporting them to identify their needs, set goals, and help them to implement their personalised health and care plan.You will support patients to self-identify existing issues and encourage proactive prevention of new and existing illnesses. This approach is based on using strong communication and negotiation skills and supports personal choice and positive risk taking.

Main duties of the job

Here are some of the main duties of the role will see the successful post holder utilising their specialist skills to

  • You will onboard patients to our Pain Management Programme this involves calling patients, setting expectations of the programme and booking them into appropriate slots. This equate to approximately 7.5 hours per week.
  • Identify and engage effectively in a non-judgemental and respectful manner with members of the community from all backgrounds, abilities and ages, listening and interpreting complex health information into understandable terms about health and health improvement.
  • Assess the clients abilities and preferences, thinking laterally and then advising and supporting them in a variety of areas/activities being and sensitive to barriers to adopting a healthier lifestyle such as affordability, accessibility and life circumstances.
  • Encourage people in choices and actions that are acceptable and achievable to them, while being aware of cultural and socialconsiderations in regard people's lifestyles
  • Conduct review of a clients circumstances and challenges to determine the nature and extent of their non-clinical needs.
