Job summary

We are seeking a compassionate and resilient experienced Palliative Care Nurse or experienced paramedic (1 year Maternity Cover) to join our team as a Senior Practitioner in palliative care.

We are looking to recruit into a new innovative post split between 2 teams, Hospice at home and the In-patient unit; this post will enable you to develop holistic skills in managing those towards the end of their lives, building relationships, and supporting patients and their families.

Main duties of the job

Would you like the valuable time to dedicate in providing the highest level of care, symptom management, support and advice whilst building upon your own skills and being supported with continued professional development?

With support from the multidisciplinary team you will play a key role in prioritising workloads and collaborating with team members to meet the unique needs of patients and their families.

This role offers 30 hours across weekdays and weekends both late and early rotas with a 50:50 split between the 2 teams.

About us

Isabel Hospice has been providing free palliative care to its patients and their families since 1982, providing support and care to people in eastern Hertfordshire who are affected by life-limiting illnesses through a range of different services.

We offer annual leave of 27 days plus bank holidays (pro rata) and rising with service to 33 days, plus pension and life assurance scheme. If you are currently working within the NHS or contributing to the NHS Pension scheme through another employer, you will be able to continue with this pension membership and we also honour long service NHS long service.

Main responsibilities

To be responsible for assessing, delivering and reviewing high standards of holistic, individualised care, support and symptom management to patients and their families in their own home or the IPU at the end of life in accordance with Isabel Hospice working practices.

To monitor the quality of care, promoting high standards in accordance with the policies of Isabel Hospice and the Care Quality Commission standards.

Provide information, advice and support to patients, families and carers face to face and via the advice line and to be aware of escalation procedures.

Demonstrate interpersonal skills that promote clarity, compassion, empathy, respect and trust.

To work as a member of a multi-professional team providing and coordinating 24/7 care for those dying in the Community and IPU settings.

Working as part of a rota covering early and late shifts which will be allocated. The IPU shifts would be consecutive as would the H@H shifts, unless service requirements dictated otherwise.

Clinical responsibilities

Undertake holistic assessments, including dynamic manual handling risk assessment, taking into account the risk associated with potential unsuitable environments.

Advise patients in relation to their illness and their families and carers on their specific needs and to enable expression of preferences and wishes in relation to future care in a safe environment.

Assess and manage symptom control with support from the Multidisciplinary Team. Pre-empting preventable, physical symptoms and reducing psychological distress.

Maintain and promote effective and efficient working and communication with other members of the multi disciplinary team across community and hospitals, care homes, voluntary private and statutory agencies. To actively promote high standards of care in all settings.

Promote privacy, dignity and respect of all patients, their families/carers and all staff.

Provide, implement and evaluate individualised care recorded in the care plan and when required participate in multi professional meetings, acting as patient advocate and representing aspects of the patients management.

Understand psychological issues as well as family dynamics affecting terminally ill patients and their families, providing support and advice specific to their needs.

Always maintain a high standard of infection control, keeping up to date with infection control procedures.

Ensure clear, concise, accurate records are made and that all communication is maintained in relation to care delivery adhering to local & national guidance.

In the absence of the Lead Nurse in H@H assist with the triaging of referrals and urgent calls during working hours, providing a rapid response as the need arises, working closely with the Hospice at Home Administrator.

Supervise Health Care Assistants and monitor quality of care, promoting high standards in accordance with policies of Isabel Hospice and Care Quality standards commission.

To participate in audits, as required.

Act as a role model to junior staff to support their development.

Promote evidence based practice and assist with implementation of changes to the service as required.
