Job summary

We have a vacancy for Health Care Assistant to join our Hospice @ Home Team based at our Head Office in Welwyn Garden City.

Based in Welwyn Garden City with travel throughout eastern Hertfordshire, this role is 30 hours per week over 4 days.

Main duties of the job

You will join a small, friendly, dedicated team providing holistic care to patients, families and carers in the community, visiting patients in their own homes and providing high quality, individualised care. This will include assisting with nutritional needs, helping to move and mobilise patients and providing personal care. You will also offer practical and emotional support to patients who are nearing the end of life and help to enable and support family and friends to continue to care for the patient, ensuring effective communication with other members of the primary care team.

This year we are celebrating providing 40 years of service to the patients of eastern Hertfordshire and continuing in our vision of providing outstanding palliative care for all.

Main Responsibilities

To work as part of a multi-disciplinary team providing holistic care for patients and their families requiring end of life care in the community. This post is a lone worker.

Day to Day Operations

To provide physical, social, emotional, spiritual, individualised, high quality care to End of Life patients and their families in their own home, respecting their privacy and dignity at all times.

Understand the role of the Volunteers and work with them in the provision of care for patients and their relatives in the community setting.

Involve and enable the family and friends to continue caring for the patient and support them as appropriate.

To refer to the Hospice at Home Lead/Isabel Nurse Specialist any symptoms, concerns, changes or deterioration in a patient’s condition.

To refer the patient to out of hours community district nursing / medical services, and seek advice via the Isabel Hospice Advice Line when appropriate

Help, move, position and mobilise patients using the principles of safe practice, following the Hospice policies and procedures.

Assist with the nutritional needs of the patient including food preparation, observing food safety regulations

Provide support and basic information to patients, families and carers, including specific care needs.

Communicate effectively with members of the Primary care team and other professionals.

Be flexible in working patterns due to the unpredictable nature of referrals to the Hospice at Home service and the needs of the patients and their families.

Take responsibility for his/her safety and be familiar with matters relating to health and safety management, reporting any potential risks to life or property immediately in accordance with the hospice’s policies and procedures.

Qualifications

Essential

  • Education / Qualifications
  • NVQ Level 2 in Care or equivalent and willing to train towards Level 3.
  • Willing to undertake in-service training/education as appropriate, in order to enhance individual skills including the Care Certificate.

Personal attributes

Essential

  • Awareness of physical and emotional needs of patients suffering from a life-limiting disease, as well as the needs of their relatives and carers.
  • Car driver and owner essential

Experience

Essential

  • Previous experience of working in a Health Care Assistant role, providing personal care, preferably in the Community.
  • Clear understanding of euthanasia law.
  • Able to respond appropriately to the changing needs of the patient.

Desirable

  • Previous experience of working with terminally ill patients preferable

Skills/abilities

Essential

  • Good verbal and written communication skills and ability to enter notes on patient computer held record.
  • Able to work independently in the delivery of patient care under the indirect supervision of qualified staff.
