When you join the Integrated Urgent Care team in Suffolk and North East Essex (SNEE) you can! Practice Plus Group are looking for clinicians for the Unscheduled Care Coordination hub (UCCH) in Suffolk and North East Essex. The UCCH vision is to ensure that People receive the right care, in the right place, first time, every time as outlined in the Government paper Next Steps to Put the People at the Heart of Care To be successful in this role, you will need to have good knowledge of local urgent care, primary care and secondary care pathways and infrastructure and the ability to work collaboratively with SNEE system partners to help deliver better quality of care for patients. This is the perfect opportunity to use and develop your skills and expertise to provide quality, evidence-based health care to meet the immediate needs of the Suffolk and North East Essex patient population. The service is operational 7 days a week, 0800hrs-2000hrs with opportunities for full or part-time work (either salaried or bank).
Main duties of the job
- Working face to face as part of a multi-disciplinary UCCH team to identify cases held in the East of England Ambulance Service (EEAST) queue that may be better served by other services
- Completing remote consultations with patients and healthcare professionals
- Making onward referrals to system partners to manage patients care needs closer to home, utilising community services most suited to their needs
- Work with members of the UCCH to identify and suggest service improvements with a focus on quality assurance and patient safety
- Act as a representative and ambassador for Practice Plus Group at all times
About us
- To work within the professional Code of Conduct as defined by the NMC or HCPC
- Current experience of working in urgent care, primary care or community care
- Educated to Masters level (or equivalent)
- Non-medical prescribing qualification with at least 6 months experience consulting and prescribing independently
- Ability to work within an MDT to make patient-focussed management decisions
- Evidence of regular engagement in evidence-based practice
- Willingness to undertake appropriate personal and professional development and keep up to date with mandatory training
- A clear understanding of remote consulting and appreciation of the challenges this can present
- An expectation to participate in regular performance review
