When you join the multidisciplinary team in our Out Of Hours service (located in various sites across Surrey and Guildford) as self-employed, bank or salaried you can. By bringing together NHS 111 call centres, clinical assessment and other urgent care services, we enable patients to access the right care, when they need it. Its better for everyone. This is an ideal opportunity for an ambitious and committed Advanced Nurse Practitioner who can independently prescribe. Its a chance to simplify and speed up the way patients receive care by assessing them, planning their care, administering treatment and discharging or referring them to other healthcare professionals or external agencies, via primary care centres and home visits. You will also be using our electronic prescribing system. Based in our various Out Of Hours (OOH) services, youll be working autonomously within your sphere of competency as part of a friendly and close-knit multi-skilled team that includes GPs, registered nurses, UCPs, mental health and dental nurses, prescribing pharmacists and pharmacy advisors. We can offer this role as either bank or self-employed.
Main duties of the job
- Facilitate clear patient pathways through primary, community, secondary and tertiary care.
- Take a person centred approach to service delivery, based on clear understanding and experience of the modes for service delivery.
- Be responsible and accountable for assessing health and / or social needs of patients within the PCC setting, using agreed clinical protocols and clinical guidelines software.
- Be able to incorporate the nurses role in health education within the unscheduled care environment.
- Maintain and develop clinical competence and knowledge of care delivery within PCC setting using both formal and informal training methods.
- To work closely with the medical practitioners in the provision of quality, evidence-based care to patients within the PCC setting or in patients own home
Additional for Advanced Nurse Practitioners
- Hold a virtual case load and be accountable for delivery and integrated, quality patient care working autonomously.
- To take a lead role in the provision of Palliative care and other specialist nursing services within the unscheduled care environment.
- To provide holistic patient care within the unscheduled care setting from assessment of presenting condition through to diagnosis and initial treatment including the prescription of medication within agreed clinical guidelines and protocols.
- To hold Nurse prescribing certificate and be competent to prescribe for patients whilst working autonomously.
About us
What you will need:
- Recent experience as an autonomous advanced nurse practitioner in a primary/urgent care environment, with a proven ability to see, treat and discharge patients of all ages with unscheduled presentations.
- A patient assessment qualification, independent prescribing qualification and NMC PIN.
- Excellent listening and communication skills, with good IT and keyboard skills.
