We have an exciting opportunity for an Advanced Nurse Practitioner to join our Clinical Hub team within our West Midlands Region.

In this role, you will have the opportunity to bring your advanced nursing skills to an organisation dedicated to making a difference to peoples lives on a daily basis.

The Clinical Hub is a brand-new innovation aimed to enhance the clinical support provided to our prisons and patient groups.

As a Registered Professional youll be an integral part of the Clinical Hub team by bringing your advanced clinical skills, Leadership, knowledge and passion for care. The Clinical  Hub team will provide advanced clinical service virtually and face to face to HMP Oakwood, HMP Brinsford, HMP Featherstone and HMP Stafford. The team will consist of an Advanced Clinical Practitioner, Specialist Nurses and Healthcare Assistants.

This position is full time, working Mon to Fri.

Youll receive an annual salary up to £60,000 per annum - DOE

Main duties of the job

As an Advanced Clinical Practitioner, your responsibilities will vary. You will

  • Complete Comprehensive Geriatric Assessments (CGA), diagnosis, treatment and evaluation of care.
  • Communicate and work collaboratively with the interdisciplinary teams across sites to supporting the delivery of the Frailty framework.
  • Provide clinical education, leadership and upskill the local workforce.

Qualifications & Experience required

  • Registered Nurse Adult, Mental Health with current NMC registration / Registered Allied Healthcare Professional with current HCPC registration.
  • Independent / supplementary prescriber qualification.
  • MSc in Advanced Practice / relevant practice experience.
  • Completion of British Geriatric Society Tier 3 Frailty or willing to work towards.

Following successful re-tender last year, we are transforming the West Midlands HiJ region to include a unique Regional Specialist Clinical Hub.

The team, based at the West Midland Regional Office, will run independently and cover all three of the Wolverhampton sites initially, with an intention for expansion across the West Midlands region.

The Advanced Clinical Practitioners will commit to the development and delivery of a frailty framework. Completing Comprehensive Geriatric Assessments (CGA) demonstrating critical thinking in a safe clinical decision-making process. Provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent expert holistic care, including assessment and diagnostic skills. Communicating and working collaboratively with the interdisciplinary teams across sites to meet the needs of patients, supporting the delivery of the Frailty framework, and providing clinical education and leadership, upskilling the local workforce.
