The East Cornwall Primary Care Network is a leading innovator in the new and developing Primary Care Network model.
We are looking for an enthusiastic, innovative and forward thinking Programme Manager to work with and across our Primary Care Network, they will be a key member of the PCN leadership team and will oversee the delivery projects across the PCN.
Main duties of the job
The role will involve managing budgets for programmes of work related to each project and to liaise with the IM&T lead and strategic manager to ensure a smoother running of all projects. The role will also involve supporting the Strategic Manager in the delivery of the PCN strategic business plan. The Programme manager will provide inspired leadership within the PCN and will assist in the development, delivery and review of operational policy and procedures.

The East Cornwall locality General Practices have formed a provider company Kernow Health East Ltd which will operate the East Cornwall Primary Care Network (PCN). The neighbourhoods you will be recruited to include the Saltash Practices (Saltash Health Centre and Port View), the Rame Peninsula (Rame Group Practice), St Germans (Quay Lane Surgery), Liskeard (Oaktree and Rosedean) and Looe (Old Bridge Surgery). You will be based across the localitys GP practices providing clinical pharmacy support as part of the practice clinical teams, working either in one or more practice(s) or remotely as part of a clinical hub.

OVERALL OBJECTIVES
This will include:
- Linking with the Strategic Manager to deliver on key locality plans aligned with the localities Primary Care Network vision, as well as seeking opportunities to align with the Integrated care system (ICS) where appropriate.
- Provide and receive sensitive patient level details and complex financial data as part of the multiple programme of work being developed by the PCN, specifically in the Directed Enhance Service Specification (DES)
- Hold a delegated budget for key programmes of work for the East PCN
- Provide monthly updates to the board of directors on key programmes of work, specifically in relation to the multiple projects being developed in ECPCN.
- Maintain a fully costed project report, detailing key milestones, task and finish group achievements and financial implications including any risks and mitigations.
- Provide inspired leadership for the organisation.
- Support the Strategic Manager in developing novel approaches to policy planning and strategy decision making.
- Strong focus on programme management, planning and delivery for the locality priorities.
- Working with practices to ensure that they are supported towards a sustainable solution for the medium and longer term.
- Supporting practices to develop local solutions and delivery models that focus on place-based systems of care and primary care networks.
- Working with system partners to ensure we have aligned priorities as an integrated care area (ICA), That ae aligned to our projects.
- In partnership with the SM and ICB comms lead, develop a communication strategy to support the ECPCN practices in understanding the more about the work of the operations team and the projects we are developing.
PRINCIPLE RESPONSIBILITIES
SERVICE DEPLOYMENT AND DELIVERY
- Assist in the development of the strategic direction for the Primary Care Network vision to ensure that all strategy developed is integrated around the patient (place), aligned to key programmes of work.
- Support General Practice to identify, evaluate and take forward new potential business development opportunities, related to the delivery of the DES.
- Support the development of business proposals and formal business cases for investment. Work with clinicians and managers to help them engage in the business development agenda and ensure they are supported and encouraged to spot and pursue income generation opportunities.
ANALYTICAL AND JUDGEMENT SKILLS
- Analyse and assess highly complex performance data, interpreting the data/information to generate options and support long-term strategic plans.
- Conduct impact analyses, assess change readiness and identify key stakeholders.
- Interpret data for various purposes to feed into service, delivery and strategy
- Exercise immediate judgement and decision making in relation to urgent highly complex matters, facts and figures and situations requiring analysis, interpretation and comparison of a range of options.
QUALITY IMPROVEMENT AND REDESIGN
- Support work with clinicians, managers and other staff across the health community, to develop and test new ideas and approaches that could bring about increased quality and efficiency within general practice.
- Develop links with external agencies/bodies to maintain a working knowledge of quality improvement and modernisation strategies, programmes and tools and techniques.
- Support identified quality improvement work streams through the use of skilled facilitation and project management techniques.
PARTNERSHIP WORKING
- Develop and maintain good relationships with practice managers, practice staff and system wide colleagues.
- Work closely with the Training the hub, hosted by Kernow Health CIC.
- Ensure appropriate agreements are in place for collaborative working and data sharing/protection, working in partnership with the IM&T Lead
PERFORMANCE MANAGEMENT
- Ensure processes and procedures are in place to help deliver the required internal and external performance and quality outcomes.
- Identify and manage anticipated resistance.
- Ensure monitoring systems are in place to enable regular tracing of quality across all of the programmes of work.
- Identify, analyse, and prepare risk mitigation tactics in partnership with all project leads.
- Define and measure success metrics and monitor change progress.
- Ensure appropriate timely data collection mechanisms are established to support business, performance and quality analysis.
