Job summary
We are seeking a skilled, compassionate, and experienced Qualified Practice Nurse to join the East Surrey Division of Modality Partnership on a part-time basis at Townhill Medical Practice for 27 hours a week. This is far more than a traditional nursing role you will play a vital part in delivering high-quality, evidence-based care, preventative health services, and supporting innovation in primary care.

Main duties of the job
This
role focuses on the delivery of high-quality,
evidence-based nursing care to patients with chronic
conditions, alongside the provision of preventative
healthcare to the wider practice population. You will use your
clinical expertise and professional judgement to assess, plan, implement, and
evaluate patient care, demonstrating critical
thinking, accountability, and excellence in every interaction.
Working innovatively and flexibly, you will contribute to service improvement
and support the effective delivery of care across the practice. The role
requires commitment, adaptability, and the ability to work resourcefully as
part of a multidisciplinary team, ensuring safe, effective, and timely care in
line with our CARE values.
