We are looking for a Medicines Care Coordinator to join our team at The Jenner Practice full time. You will be a key member of the multidisciplinary team, working closely with the Lead Pharmacist and PCN Manager to support the safe, effective, and efficient use of medicines while contributing to service improvement and performance delivery.

Main duties of the job
This is a varied and dynamic role supporting medicines optimisation across the PCN. You will work collaboratively with clinical and operational colleagues to help deliver agreed KPIs and performance targets in line with Standard Operating Procedures (SOPs) and the PCN business plan.
The role is not a standard 9-5 position and requires flexibility, initiative, and the ability to work resourcefully as part of a wider team to ensure tasks are completed effectively.
Key responsibilities include:
- Working with the Lead Pharmacist and PCN Manager to support delivery of medicines-related KPIs
- Coordinating medicines-related processes across practices to ensure safe and effective patient care
- Supporting communication between practices, patients, and external healthcare organisations
- Contributing to innovation, service improvement, and harmonisation of ways of working
- Managing workload effectively while working to tight timescales
- Handling sensitive situations professionally and compassionately
