Job summary
Modality
Partnership AWC Division has a part-time vacancy for 20 hours per week, fixed term for six months, for a Nurse Care Coordinator, to support with the E-falls project.
The postholder will be a high performing employee who will work as part of a
multi-disciplinary team across AWC Division. We are seeking dedicated

Main duties of the job
The role will ensure patient health and care planning is timely, efficient, and
patient-centred. This is achieved by bringing together all the information
about a patients identified care and support needs and exploring options to
meet these within a single personalised care and support plan.
The role is not
a regular 9am-5pm job; the job requires innovation, flexibility and commitment
and the postholder will be required to work resourcefully as part of the team
to ensure tasks are completed.
