Modality
Partnership Walsall Division has a full-time vacancy for an Advanced Care Practitioner for 37.5 hours per week working at Pleck Health Centre. The
successful candidate will play a vital role in delivering high-quality patient
care, utilising advanced clinical skills to assess, diagnose, and manage
patient conditions across the Walsall Division. This position requires a strong
foundation in anatomy knowledge and the ability to administer medications
effectively. We are seeking dedicated
postholders to join our team and who can display our CARE values: Commitment,
Accountability, Respect and Excellence. Salary for this role is starting from
£55,000 per annum WTE. Trainee Advanced Care Practitioners are welcome to
apply and starting salary for them would be £46,000 per annum WTE.

Main duties of the job
The role is an all-rounded,
clinical ACP role; the postholder will need to confidently evidence and utilise
key skills such as ability to work on own initiative, ability to develop an
evidence-based, personalised, integrated health and social care plan in
partnership with the individual, carers, relatives and health care
professionals, based on a full assessment of medical, nursing and social care
needs, and demonstrate ability to act as an autonomous advanced practitioner
providing a comprehensive clinical assessment service to
patients/carers/families resulting in safe, appropriate clinical decisions. The
postholder will work in collaboration with local divisions (including
Divisional Boards) and the National Modality Business Support Team to deliver innovative
ways of working to meet the strategic aims of the organisation.
The role is not
a regular 9am-5pm job and is not a pure patient facing role; the job requires innovation,
flexibility and commitment and the postholder will be required to work on
quality improvement as outlined in the job specification and work resourcefully
as part of the team to ensure tasks are completed.
