Modality Partnership
Hull Division has a full time vacancy for a Health Care Assistant with
phlebotomy skills to work across Alexandra Road and New Hall Surgeries. We are seeking dedicated postholders who will
work collaboratively with the general practice team to meet the needs of our patients
through the delivery of safe patient care. This role has
an essential criterion of holding a level 3 Healthcare Support Worker qualification and is not a training role.
The post holder
will support as part of the wider nursing team across Modality Hull Medical
Practices and provide weekly visits to care homes to promote the health and wellbeing of our patients by assessing their
needs, providing pre-screening activities and health care for chronic diseases,
health education advice and support, implementing requirements in line with
patient requirements, community, or other settings, maintaining clinically
accurate records during clinics.
This job is
suitable for individuals who can demonstrate safe and effective patient interactions
ensuring the delivery of high-quality patient care whilst working alongside the
wider nursing team and practice team.

Main duties of the job
This is a
hands-on patient facing role; we are seeking individuals to join our team who
are caring, compassionate and confident, and who demonstrate effective
communication, excellent customer service skills, good IT skills and the
ability to support patients with interventions and/or queries. The postholder
will be a key member of the practice team working under the line management of
registered Practice Nurses. The postholder will be responsible for undertaking
patient health checks to include observations such as BP monitoring, ECG
monitoring, height and weight, phlebotomy, and provide health and wellbeing advice
and promotion such as smoking cessation advice and signposting to requirements
in line with the patients health needs.
