Job summary

Modality Partnership (Airedale, Wharfedale and Craven Division) has a full time vacancy for Practice-based Wound Care Specialist Nurses. The postholder(s) will be a registered nurse with qualification/skills in the management of complex and chronic wounds. Working across Modality Airedale, Wharfedale and Craven Division one post to cover across Craven sites: Fisher Medical centre and Gargrave, Cross Hills, Steeton and Silsden and one to cover Airedale sites: Haworth and Oakworth, Kilmeny and Long Lee and Farfield and Holycroft. Responsible for the delivery and management of complex and chronic wound care to the whole practice population. Main duties of the job

The role is both the delivery of evidence-based practice for patients presenting with complex and chronic wound care requirements, and the provision of preventative health care to the practice population. Responsible for the care delivered to their patients, demonstrating critical thinking and skills in clinical decision-making in the management of patients.

The role is not a regular 9am-5pm job; the job requires innovation, flexibility, and commitment and the postholder will be required to work resourcefully as part of the team to ensure tasks are completed.

Essential

  • RN qualification, NMC pin holder with no restrictions.

Desirable

  • Nurse mentor qualification i.e., facilitating learning in clinical practice, Advanced qualification in wound care, Nurse prescriber qualification, Experience and formal qualification in the management of long-term conditions.

Experience

Essential

  • Experience of complex and chronic wound care/leg ulcer management, with evidence of compression bandage application and doppler training, Experience using system one, Able to demonstrate sound nursing experience using evidence-based practice.

Knowledge

Essential

  • Speciality in wound care and the management of defined chronic wounds, Knowledge of diagnosis and management of patients with chronic conditions, Knowledge of accountability of own role and other roles in primary care, Knowledge of health promotion strategies, Knowledge of local and national health policy, Wider health economy awareness, Knowledge of clinical governance issues in primary care, Knowledge of patient group directions and associated policy, Knowledge of current immunization schedule, Knowledge of local and national guidance and policy relating to all relevant areas of own practice.
