Modality
Partnership Hull Division, have a full-time vacancy for a
Practice Nurse with interest in long term condition management. The postholder will work at Alexandra Road and Diadem Practices, and may be required to provide cross site cover across
the Division.

Main duties of the job
Within the role
of the Practice Nurse, you will provide care and management of patients with
long term health conditions such as Asthma/COPD, as well as providing all
aspects of nursing care duties such as wound care, adult and childhood
immunisations, travel vaccinations, cervical cytology screening, contraception
and sexual health screening including promoting a healthy lifestyle.
All relevant and necessary training
would be given to the right candidate who has a willingness to learn and
undertake training
