Main duties of the job

The post holder will work 4-6 sessions of core primary care clinical work and 1 session of lifestyle medicine/organisation development work. The role is a patient centred one although there is an opportunity to work remotely for some sessions. Our Diabetes Reversal Programme is being delivered via virtual and face to face group consultations. We will provide you with the supervision necessary to safely de-medicate diabetic patients and lead group consultations. You will be trained in facilitating an holistic approach to patient health and well-being, emphasising the importance of movement, sleep, stress management, diet and exercise. For those of you who are eligible we will support your application for a post-CCT Fellowship programme.

This job is suitable for individuals who are compassionate, patient focused and have a real passion for primary care, and working within a larger GP super partnership, who are dedicated to making real changes to the health care services provided within Primary Care to improve patient care and the work life balance for our staff.

About us

We are the largest GP super-partnerships in the UK, with a list size of 450,000 patients and with a multi-disciplinary workforce of 1500 staff. We are unique, always looking at ways to improve our delivery of services through the implementation of new and innovative solutions that we can scale across the organisation. Recent innovations have included the use of robots to facilitate the remote filing of blood results and the development of document management team. These new initiatives are underpinned by our governance team to ensure patient safety.

We are seeking dedicated postholders to join our team who will embrace our partnership values: Commitment, Accountability, Respect and Excellence (CARE).
