Job summary
Were looking for a Nurse Associate (Band 4) to join our North Wiltshire Hospital at Home team.
This is an exciting time to come on board as the service undergoes transformation, offering a chance to be part of an innovative and supportive team.
Youll be based at Chippenham, Malmesbury, or Savernake Hospital, working between 8am6pm, Monday to Sunday (flexible shifts considered).
As a Nursing Associate, youll bring your skills and compassion to deliver high-quality, person-centred care in patients homes. Youll have completed the Nursing Associate Foundation Degree and work within the NMC Code, supporting individuals across the lifespan.
In this role, youll carry out a range of delegated duties, guided by a competency framework, and work independently within your scope of practice, always supported by a registered practitioner and in line with policies and procedures.
Main duties of the job
As a Nurse Associate you will:
- Work in care teams with other professionals, providing practical help by delivering aspects of nursing care to vulnerable and often highly dependent patients.
- Use sound clinical reasoning skills when delivering care and therapy interventions
- Deliver a delegated workload against the needs of patients and service
- Treat all individuals with respect and maintain privacy, dignity and confidentiality
- Commit to personal and professional development
- Organise, prioritise and adjust own workload
- Accurately and effectively report on care undertaken and document changes in condition, maintain records, and ensure that there is feedback to the appropriate or responsible healthcare professional.

As a Nurse Associate Band 4, you'll be part of our valued team in our North Locality Hospital at Home service.

Essential
Registered as a Nurse Associate with the Nursing and Midwifery Council (NMC)
Experience of working within a clinical setting
Knowledge of relevant legislation and its impact on current practice
Experience of working within a multi-disciplinary team
Effective verbal and written communication
Excellent interpersonal skills and able to build effective working relationships with both clients and colleagues
An ability to respond to people in distress
Car driver, with access to own vehicle.
Desirable
Previous experience in a care or health setting
Experience of managing patients with complex needs in the community.
Understanding national drivers and guidance relevant to Hospital at Home, or willingness to learn.
