Job summary
Are you an experienced Advanced Nurse Practitioner or Nurse Practitioner with an independent prescribing qualification and a strong background in urgent or community care?If youre looking for a flexible part-time role where youll work autonomously but also be supported by a skilled and friendly MDT, then this could be the right fit for you.
Youll play a vital role in our Urgent Care Response service, helping patients stay safe at home, avoid unnecessary hospital admissions, and access timely, person-centred care.
Main duties of the job
What youll do
- Carry out advanced assessment, diagnosis, and treatment for patients with undifferentiated conditions (including minor illness and injury)
- Use your prescribing qualification to deliver safe, timely, and effective care
- Support the Urgent Care Navigation Hub by triaging referrals and directing patients into the right pathways
- Provide visible leadership, clinical guidance and supervision to nursing colleagues
- Contribute to service development, audits and governance work to continuously improve care

As a Band 7 Advanced/Extended Nurse Practitioner, youll be part of our team primarily based at Livingstone Community Hospital, Dartford, with some travel to the Urgent Care Navigation Hub, Strood.
