Wilton Community Team are seeking a Band 6 Community Nurse.

This is a full time Fixed Term role. Part time can be considered for the right candidate.
As a Specialist Community Nurse, you will:
Provide nursing care, treatment, support, and advice to individuals in their own homes or outpatient settings.
Promote patient independence and wellbeing, working within national and local frameworks, professional guidelines, and organisational policies.
Deliver safe, person-centred care to a diverse caseload of patients with both short-term and long-term conditions.</p><p>Take clinical leadership responsibility for peers, junior trained staff, and support workers within your discipline.
Offer expert advice and guidance to other health and social care professionals.
Act as a key member of the team, contributing to service development and maintaining high standards of care.
What Were Looking For
A registered nurse (Adult) with current NMC registration.
Significant post-registration experience in a community or primary care setting.
Main duties of the job
Ensure own clinical credibility through professional practice, regular updating and maintaining clinical skills.
To undertake specialist and complex assessments and care planning.
To prescribe medication and other items safely and appropriately within the context of national and local policies, guidelines and formularies.
Give care and support to terminally ill patients and their carers/relatives, including discussing prognosis and preparing for death. Ensuring other relevant agencies are involved.
To have sound knowledge of infection control, to reduce risk of health care acquired infections.
To maximise the use and skills of junior staff by supporting appropriate delegation of tasks by self and others.
To deliver urgent care/planned care/care to people with LTC / post-acute needs / older people.
To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making
To deliver clinical care for a variety of patients, some with extremely complex needs
To complete assessments for patients with continuing healthcare needs and provide ongoing clinicalcase management.
