Job summary
As a Discharge Liaison Therapist/Nurse, you will play a key role in delivering high-quality, person-centered care and services to support individuals and teams. Working within a collaborative environment, you will provide essential services tailored to meet the unique needs of those we serve.
Based in Bath, you will have the flexibility to work within a model that supports both on-site and hybrid working arrangements, depending on the nature of the role. Outline of Provisional Job Schedule:
Working pattern: 22.5 hrs per week, 7 day working Including weekends, bank holidays and Christmas
Main duties of the job
The key responsibilities of the Discharge Liaison Therapist or Nurse are as follows:
Work collaboratively with the integrated discharge team, following the Discharge to Assess model, to ensure patients are on the correct discharge pathway and well-prepared to leave acute or community hospital care.
Conduct thorough, holistic assessments that consider each patients medical, physical, social, and emotional needs. Apply clinical reasoning and evidence-based practice to guide interventions, making sound judgments both in person and over the phone, often in complex situations requiring prompt decisions.
Partner with patients, families, and the multidisciplinary team to create and implement safe, person-centred discharge plans. Promote independence, encourage patient involvement, and maintain open, effective communication to support seamless care. Take responsibility for case management, including accurate documentation and risk assessments.
Utilise IT systems effectively to avoid delays in care, and support the team with professional advice, day-to-day coordination, and data-driven decision-making.
Apply advanced problem-solving skills to understand complex issues, evaluate options, and determine the best course of action with limited information.
