Were looking for a compassionate Band 5 Learning Disabilities Community Nurse to join our Specialist Learning Disabilities Complex Health Needs Service.
Our Learning Disability, Autism and Neurodiversity division supports adults across Bath and North East Somerset, Swindon, and Wiltshire. We are committed to person-centred care, helping individuals lead meaningful lives with choice, control, and inclusion in their communities.
As part of our multidisciplinary team, you'll work alongside experienced nurses and clinical specialists to provide assessments, therapeutic interventions, and ongoing support for people with complex needs including those with challenging behaviours, mental health conditions, and profound disabilities.
This is a rewarding opportunity to be part of a supportive, innovative service, with chances to collaborate across health and social care settings. The post is full-time (37.5 hours/week), Band 5 (£29,970 £36,483), with preceptorship available through the Learning Enterprise.
Main duties of the job
This role offers a rewarding opportunity to work closely with health and social care professionals, supporting individuals with complex needs such as dementia, epilepsy, behaviour management, and mental and physical health conditions. You'll contribute to key areas like hospital admissions and discharges, Continuing Health Care.
You'll work alongside a wide multidisciplinary team across primary, secondary, and tertiary care, and engage in eligibility assessments with experienced colleagues.
You'll help promote health and well-being by reviewing care plans with individuals and their carers, encouraging independence, and supporting access to mainstream health services to reduce health inequalities. Your responsibilities will include managing a caseload, supporting care planning, medication management, and offering education around healthy lifestyles.
The role also involves supporting behavioural and epilepsy reviews, delivering training in areas such as dementia and bowel management, and feeding back clinical observations to relevant professionals.
You'll participate in multi-agency meetings, safeguarding work, and contribute to pathways like dementia, mental health, and end-of-life care. Regular supervision, peer forums, and ongoing professional development are provided, along with involvement in audit and data collection for local and national reporting.

- Current registration.

- Ability to work autonomously.
- Knowledge of MCA
Desirable
- Excellent skills in accessible information.
- Problem solving skills.
- Ability to evaluate and reflect on practice.
- Experience of Safeguarding.
- Experience of epilepsy; challenging behaviours; CHC.
Other Requirements:
- Driving licence and own care as post is community based covering remote and rural locations.
- The role is full time 37.5 hours per week Mon Fri 9.00 am 5.00 pm
