Health Improvement Practitioner - Diabetes

Job summary

As a Health Improvement Practitioner you will work in our BaNES Integrated Neighbourhood team and be a source of specialist advice and support for our XPert diabetes service for some of the most vulnerable people living in Bath and Northeast Somerset. Although your main focus will be supporting people to manage their diabetes diagnosis, you will at times to a lesser extent, support people with weight management, healthier cooking and physical activity in-line with our person-centered approach to improving peoples overall health and wellbeing.

This work will be carried out on a one-to-one and group basis within different settings throughout Bath and Northeast Somerset..

As a 'Health Improvement Practitioner,' you will work 22.5 hours per week where your working hours will be spread over 3 days FTC until March 31st 2026. A typical working week is Monday to Friday but may include some evenings and weekend.

Main duties of the job

As a Health Improvement Practitioner within HCRG Care Group you'll join a highly skilled team who are focused on service excellence.

This role focuses on the design, delivery, and evaluation of the Xpert diabetes programme across Bath and North East Somerset. You'll provide structured support to individuals living with diabetes through a range of settings including group sessions, one-to-one support, telephone and digital engagement.

You'll work closely with local services, to ensure continuity of care for those with diabetes. There's a strong focus on supporting individuals in areas of health inequality, contributing to wider health initiatives like weight management, nutrition, smoking cessation, and physical activity.

Key Responsibilities:

  • Deliver and monitor the Xpert diabetes programme in various formats and settings

  • Maintain a busy caseload and provide tailored support to individuals

  • Train and support other professionals in diabetes care and related interventions

  • Develop and share health promotion materials and resources

  • Support wider public health campaigns and contribute to related health programmes

  • Assist with programme evaluation, data collection, and reporting outcomes

Additional Information:

  • Based at Kempthorne House, with travel across Bath and North East Somerset

  • Hybrid working is expected, with office, community

  • Some evening and weekend work may be required

  • Degree or equivalent in Mental Health, Wellbeing, Healthcare, Customer Service, or a related field

  • Proven experience leading and motivating teams

  • Confident in managing office systems, procedures, and digital tools (Word, Spreadsheets, Teams)

  • Skilled in partnership working and developing marketing materials

  • Experience in monitoring, evaluation, and project coordination

  • Comfortable working to deadlines and handling multiple priorities

Skills & Personal Attributes

  • Strong interpersonal and communication skills, both verbal and written

  • Organised, detail-oriented, and able to work independently or as part of a team

  • Supportive, empathetic listener with a problem-solving mindset
